We make a large effort to stay on time and are fairly successful at this. However, we do see patients both in clinic and at the hospital on an emergency basis. Sometimes, these emergencies can make us late. We ask your patience when this happens.
If you have been waiting more than 15 minutes, please let the front desk staff know. If the delay inconveniences you, we can reschedule the appointment.
We also ask that you be on time for your appointment. If you are late for your appointment, we will attempt to reschedule it for you later in the day but can not guarantee this.
Please inform the receptionist of your arrival before being seated. Since space in the waiting room is limited, we ask that you limit the number of children and friends accompanying you to those necessary for the office visit. For the consideration of the children who are ill, we do request you bring someone to supervise young children who must be left in the waiting room.
Each new patient or established patient who has not visited the practice in three years will be asked to complete a New Patient Health History. Click on the link to download this form so you can fill it out before your visit. This information is used by the physician as he or she considers and assesses your problem. Please supply all information in a complete and accurate manner.
We also ask each new patient to review the government mandated privacy policy and sign that they have received this information.
At each visit you will be asked to review your address and contact information to ensure its accuracy. If any of this information changes in between visits please let us know.
